Request for ZMI Transcript:

Graduates and former students of Zion Ministerial Institute in Waverly, New York may use this form to request a copy of their Student Transcript. All transcript requests must be made in writing, and signed by the student requesting. As of 2015, after the first complementary transcript request per calendar year, students will be charged $5.00 per transcript request.
